Through a partnership with St. Joseph hospital, we offer mental health (MH) and substance abuse (SA) services for patients with a medical diagnosis. Patients being treated at the hospital for a chronic medical condition (e.g., diabetes, hypertension, back pain, etc) get referred by their physician to get a screening for mental health. If the patient scores high in the screening tool, he/she gets referred for a MH/SA evaluation and may be asked to participate in individual therapy sessions and/or invited to participate in educational/support groups.
The IC team works at several clinics at the hospital including the Medical Clinic, Pain Management Clinic, Women’s Center, Mercy Place and Clinica Santa Teresa. Services are provided at these clinics or at Centro Multicultural office.
Other services provided by this program include:
- Case management for patients with co-occurring disorders – assistance is provided to patients who are in need of applying for insurance, Social Security Disability, or food stamps. Staff connects them with resources in the community to obtain emergency food, clothing, employment or support groups.
- Educational/informational support groups – weekly groups on topics such as how to manage stress, relaxation techniques, importance of a healthy lifestyle
- Limited transportation to group sessions and clinical appointments
- Coordination between physical and behavioral health providers – assistance with appointments with other providers
